Rachel has been captured by the aliens and is being experimented on.  She knows that her organs are being taken for transplant into an alien-human hybrid.  She manages to smuggle out this last note to her daughter via the doctor performing the experiments.  He does this favor for Rachel as his family is also being held hostage by people in the government working for the aliens.
